ماده1. موضوع قرارداد:

انجام کلیه ی امورات معمول و عرفی منزل شامل پخت وپز، ظرفشویی ،نظافت خانه مربوط به طبقات4و 3

ماده2.مدت قرارداد:

مدت این قرارداد از تاریخ01/11/1397الی تاریخ30/12/ 1397به مدت 2ماه بوده و خدمات آن بصورت تک شیفتی از ساعت 10الی13 به مدت3 ساعت می باشد.

پس از توافق برای قرارداد مذکور،در پایان اسفند ماه1397 ، تمدید یا لغو قرارداد با توافق طرفین و با اعلام هر یک از طرفین میسر خواهد بود.

ماده3:مبلغ قرارداد

مبلغ قرارداد بصورت ماهیانه 7500000ریال(هفتصدو پنجاه هزار تومان)در برابرکارکرد 1نفرنیروی خدمات میباشد. و مبلغ فوق بصورت مقطوع بوده و تا پایان قرارداد قابل تغییر نمی باشد.

ماده5:شرایط و مقررات عمومی کار

1-5-تمامی هزینه های قانونی مربوط به حقوق و دستمزد پرسنل اعم از مالیات ، بیمه تأمین اجتماعی ، حق سنوات ، حق مرخصی،پاداش پایان سال عیدی ،حق شیفت ، بیمه حوادث و هرگونه حقوق قانونی ، بر عهده پیمانکار می باشد..

2-5-تأمین تمامی وسائل و تجهیزات مورد نیاز خدمات به عهده کارفرماست.

3- 5- در صورت غیبت و یا مرخصی پرسنل ، پیمانکار موظف به تعیین و جایگزینی جانشینی واجد شرایط در محل بوده و موظف به انجام تعهدات خود می باشد.

4-5- هرگونه پرداخت خارج از مقوله قرارداد اعم مساعده ، حقوق و .... مسئولیت آن بر عهده خود کارفرماست .

5-5- در هر مقطع زمانی که کارفرما از عملکرد نیروی خدماتی رضایت لازم را کسب ننماید ، مراتب را به پیمانکار منعکس و شرکت موظف به جایگزینی شخص واجد شرایط در اسرع وقت میباشد .

6-5-خدمات انجام گرفته مطابق تقویم رسمی کشور می باشد ودر صورت کارکرد در روز های تعطیل اضافه کاری محسوب می شودکه باید کارفرما ،پیمان کار رادر جریان امر قرار دهد.

7-5-ایجاد محیطی آرام توام با آرامش روحی و روانی جهت بهره وری کارکن از الزامات این قرارداد و بر عهده کارفرماست.

8-5- انجام هرگونه ضیافت و مهمانی در ساعات ذکر شده در این قرارداد که موجب ناراحتی و شخصیت یا حیصیت پرسنل شرکت را شامل باشد، عواقب ناشی از آن بر عهده کارفرماست..

9-5- طبق گزینش انجام شده توسط شرکت خدماتی و مورد تایید کارفرما در این قرارداد از میان واجدین شرایط،سرکار خانم    فرزند      و به شماره ملی

به عنوان نیروی کار اعزامی از طریق شرکت جهت انجام خمات طبق ماده 2 در این قرارداد می باشد.

10-5- مسایلی که بعد از تنظیم قرارداد موجب مسایل کیفری شود شامل این قرارداد نمی باشد و در خصوص چنین اختلافاتی باید کارفرما و پرسنل به داد گاه صالحه مراجعه فرمایند و هزینه ای بوجود آمده شامل هزینه های تنظیم قرارداد متوجه کارفرما می شود.

11-5- هر گونه توافقات انجام گرفته بین کارفرما و پرسنل شرکت خارج از این قرارداد عواقب آن برعهده خودشان می باشد.

ماده6 : فسخ قرارداد.

6-1-عدم پرداخت به موقع مبلغ قرارداد توسط کارفرما.

6-2-تغییر محل زندگی کارفرما به نحوی که به تشخیص شرکت موجب مشکل برای پرسنل گردد.
